Insurance information
Tenth Street Pediatrics is currently contracted with many (but NOT all) PPO plans for Anthem Blue Cross, Blue Shield, Cigna, Aetna, and United Healthcare insurance (this does not include POS, EPOS, EOS, or HMO plans).
To expedite billing to your insurance company, please bring a current copy (front and back) of your insurance card. You may also email or fax us the information prior to your first visit. We will collect new insurance information at the beginning of each calendar year.
For all non-contracted PPO plans, payment in full is due at the time of each visit. We will bill your insurance company directly for you as a courtesy. Any out-of-network insurance reimbursements will be sent directly to the insured.
We are continuing to charge an annual Administrative Fee for our in-network patients to defray the costs of non-covered and non-reimbursable services that are not covered by contracted PPO plans.
